High Cholesterol with History of ASCVD

High Cholesterol with a history of Atherosclerotic Cardiovascular Disease

You may qualify if:

• At least 18 years of age
• You have prior history of myocardial infraction
• You have coronary or other arterial revascularization
• You had an Ischemic stroke or transient ischemic attack
• You are taking any cholesterol medications (Statins eg Crestor, Lipitor, Pravachol, Zocor, etc)
• Your LDL-C ≥ 100 mg/dL at last measurement or LDL-C ≥ 70-99 mg/dL with no change LLT for 4wks
