Are you at least 18 years of age and have been diagnosed with Type 2 Diabetes?

You may qualify if:

• You are taking DPP4(i) with or without Metformin at a stable dose for at least 12 weeks.
• Your Blood Pressure is less than 180/100 mmHg
• Your HbA1C is between 7.0% to 11.0%
• Your Kidney Function: eGFR more than 45 mL/min/1.73mˆ2
